DescriptionThe case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.Providence caregivers are not simply valued - they're invaluable. • 1 year of Job related experience in a healthcare setting.Preferred Qualifications:
  • Associate's Degree
  • Bachelor's Degree
  • Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire.
  • Prior experience in discharge planning, billing, utilization review, financial counseling or registration.
